FOR PATIENTS
Urology is the branch of medicine which treats the disease of the kidneys, bladder, and urinary tract in both men and women, as well as the prostate and reproductive system of the male. The urologist is a medical physician and surgeon specially trained for the diagnosis and treatment of these genito-urinary problems.
TO OUR NEW PATIENTS
It is our pleasure to welcome you to St. Louis Urological Surgeons, and to assure you that we are dedicated to providing you with the highest quality urological care in a compassionate and professional environment. Our physicians and staff understand their responsibility to you, and will strive to fulfill it in every way possible.
To simplify the process upon your first visit to one of our offices, please complete the new patient infomation forms and physical history sheet prior to your arrival. If you have questions about any of the items on these forms, our staff will be happy to assist you.
To further insure that your visit will proceed smoothly, you need to understand the requirements of your health insurance coverage prior to your visit. In particular, you will need to present a current insurance card upon arrival, and if your insurance requires a referral from your primary care physician it is your reponsibility either to bring it with you, or to insure that it has been transmitted to us. You will be responsible for all charges incurred if we are not provided with a necessary referral. We strongly encourage you to contact your insurance carrier to determine whether or not you need a referral for your visit to the urologist.
